The SF HOME Project supports
homeless and very low income
families transition to stability and
independence by providing
strengths-based case management
and comprehensive supportive services
in a home-based setting.
How It Works
Families in market rate housing receive a monthly rental subsidy coupled with home-based comprehensive case management services. This subsidy is available for up to one year. During this time, families receive intensive services to help them successfully transition off the subsidy, either by increasing their income or by securing permanent affordable housing.
Families already living in subsidized housing can receive the home-based case management services without the subsidy.
In either situation, clients work on-on-one with a case manager, addressing each family member's needs. The case manager and the client assess the family's current situation and the mutually agree on an action plan to achieve that family's goals. Families and their case managers meet twice a month, with regular home visits.
With a foundation of case management, families can access a range of other supportive services including therapy and substance abuse counseling. Parenting education and support groups are available though Compass Family Center's Positive Parenthood Project. Job readiness training and vocational counseling is offered the SF HOME's Opportunities Program.
