Program Referrals

Central City Access Point is one of three entry points in San Francisco for families experiencing or at-risk of homelessness to connect with the Family Coordinated Entry System. We are the front door referral program for shelter and housing as well as problem-solving financial and one-on-one resolution services.
Eligibility:
-
Verified homeless: car, RV, unsheltered, emergency or individual room shelter, transitional housing, couch surfing, doubled up without a lease (except if with parent or adult child), SROs, CalWORKs hotel stay, DV shelter, substance use or mental health treatment facility.

Have ties to San Francisco: stayed in SF within the last 7 days, have children in SFUSD or Daycare, or are current/former ward of the SF Foster Care system.

Have at least 50% physical/legal custody of a minor child, or are pregnant or be in the process of reunification with a letter stating that children will be returned to care within 90 days and that shelter/housing is the only barrier.

Birth certificates or hospital verifications for all minor children and/or proof of pregnancy.

Social Security Cards (if applicable)

Letter of residency if residing in emergency or individual room shelter, transitional housing, CalWORKs hotel stay, DV shelter, substance use or mental health treatment facility.

In person: 37 Grove St during operation hours to meet with a staff member for an eligibility assessment. Intake appointment will occur within 2 business days.

Email: Referrals by staff members can be sent to Megan Geary and Helen Cartagena via email. A team member will triage and contact the client within 5 business days.

Compass Childcare Support Services (CCSS) provides case management for families experiencing homelessness in San Francisco who are looking for childcare & have a child 0-5 years old.
Eligibility:
-
Family must reside in San Francisco
-
Family must have a child between the ages of 0-5 years old and not yet in kindergarten.
Referral Process:
NOTE: All referrals must be submitted through Salesforce!

Select Compass Childcare Support Services from list of programs when making a new Referral/Case in Salesforce

Enter the client's contact information and fill out referral infromation

You do NOT need to worry about other childcare intake details. Leave those blank!

SAVE and you're done!

Referral Process:
Therapy is available to any family enrolled in one of Compass's programs. Please make a referral for your client to Compass Behavioral Health Services through Salesforce. A few things will make this process go more smoothly:
Please make sure there are children listed in the case record. Unless the parent is pregnant, we cannot provide therapy for adults without minor children in their care. We can help you find resources for them at another agency. Please do not put in therapy referrals for anyone without children.
Submitting one referral per family is sufficient. Once CBHS connects with the adult, we can create additional referrals as needed.
Please put in a housing tracker. We have a contract to provide services to clients living in HSH funded sites and need to report on this. We can also serve any Compass families, but we need to know where they are living.
Please fill out the section that says "reason for referral." CBHS will learn more from the client directly, but it is helpful to give a sense of what is going on and why the client have agreed to receive therapy services.
Please ensure that your clients know you are making a CBHS referral. Therapy is fully voluntary. If the client does not want therapy, please do NOT put in a referral!
Please inform clients about expectations around therapy. At Compass, therapy is a weekly 50-minute session that takes place on the same day of the week, at the same time, and client participation is essential. Being in therapy is a commitment. Sometimes clients have an expectation that they can call their therapist whenever they feel the need to talk, but therapy doesn't work that way. Please share these general guidelines with clients prior to referring them to set expectations and ensure they have the capacity to participate.

C-Work supports parents with job readiness, barrier removal, skill building, job placement, and other resources to help client families find stable work and earn a livable wage.

Strong motivation to increase skills and find employment.

African American clients, including parents with African American children, are encouraged to apply!

CFRC provides:
Drop-In Services- basic need supplies (food, diaper, etc), information and referrals (internal and external), child watch during on-site meetings (limited until onboarding of position). Families that have been closed in all other programs can still be open with FRC to receive drop-in goods and other information and referrals if the family still has a child in their custody under the age of 18.
Family Advocacy- light touch, short term service plan + FAM, contact 1-2x per month min
Case Management- intensive service, weekly contact, service plan + FAM, contact 1x per week (prioritize families with children 0-5yrs)
Parent Education- curriculum-based parenting series (3P), workshops, parent/child interactive groups, peer support groups, 1:1 perinatal/infancy support (prioritize families with children 0-5yrs)
